The AYMES Therapy
Optimisation Resource

Here to support you and your patients.

The AYMES Therapy Optimisation Resource is designed for healthcare professionals to take a systematic approach in optimising local pathways and helping to simplify the prescribing of Oral Nutritional Supplements (ONS).

The resource has been developed to provide healthcare professionals with a flexible offering, allowing you to select the elements of the resource which best meets the needs of your healthcare organisation.

Using the GP clinical systems, the resource enables you to take a systematic and consistent approach to identify, manage and support patients more efficiently. The patients identified are those who may be considered for a review of their current ONS or those who may benefit from consuming ONS as part of their overall malnutrition management plan.

The AYMES Therapy Optimisation Resource offers access to a pharmacist-led clinical review service to support the review of suitable patients.

This resource is offered to UK based GP practices, Clinical Commissioning Groups (CCGs), Local Health Boards and Primary Care Networks, who use either EMIS or SystmOne.

OUR RESOURCES

There are three core elements to this resource, which can used inisolation or together, empowering you to access the right solutionto suit your needs. The three solutions offered include:

Patient Identification Resource

This consists of importable clinical system searches, enabling a systematic, consistent and efficient approach in identifying patients who may be at risk of malnutrition and/or suitable for a review of their current ONS prescription.

Pathway Optimisation Template

Available for Clinical Commissioning Groups (CCGs), Local Health Boards and Primary Care Networks only.

Helping to simplify the prescribing of ONS products, in line with local guidelines, utilising the functionality of the GP clinical system. The clinical system template is triggered when an appropriate patient presents, enabling the prescriber to view the local guidelines at the point of consultation.

The template also allows the prescriber to access educational tools and resources for both healthcare professionals and the patient from within the template. The template is configurable to your local guidelines and resources.

Clinical Review Service

Led by a clinical pharmacist, this service supports healthcare organisations in achieving the recommended monitoring and review of patients who have been prescribed ONS; as well as the identification of new patients who may be at risk of malnutrition and meet the criteria for ONS use but have not yet had a malnutrition management plan in place

The Patient Identification Resource only identifies patients potentiallysuitable for a review of their Oral Nutritional Supplements. It does notreplace the need for a clinical review of their malnutrition management. It is important to note that all decision making and clinical judgementthroughout this process remains with the healthcare professional.
